Coverage for resident stays will vary by individual insurance policies. Our staff will work with you and your provider in order to verify insurance coverage. Please note that pre-authorizations are
not a guarantee of payment and only your insurance carrier is able to make such determinations.
Our daily activities include, but are not limited to bingo, arts and crafts, puzzles, etc. Some of our special activities include fishing trips, shopping trips, and participating in
Bremond’s Annual Polish Days.
Our normal visiting hours are from 7 a.m. to 8 p.m., 7 days a week. Other times may be accommodated and should be arranged in advance with our facility staff.
Families should contact the hospital discharge planner and let them know you would like your loved one to be admitted to our facility following discharge from the hospital.
We encourage our patients and their families to bring items that will make their stay with us comfortable. Along with these, patients should bring any personal care items such as eye
glasses, hearing aids, toiletries, etc. and enough clothing for 5-7 days of wear and comfortable shoes.
Our current private pay rates are $124.00 a day. Private insurance, Medicaid and Medicare beneficiaries are responsible for payment of any and all applied co-insurance/patient liabilities
determined by policy/program.
Patients are evaluated on smoking safety by the facility social worker. Patients may smoke, but only during allocated times and under supervision.
Doctor visits vary on individual needs of the patients. You have the right to use your own personal physician or, in the event you do not have a current provider, we have compiled a list of independent
providers who are available to serve you. All physicians you elect, however, must be credentialed to practice at the facility and must comply with all facility policies and procedures as well as applicable laws
Yes, we have a 16-bed Alzheimer’s and dementia unit with a wander guard system.
Patients can be admitted 24 hours a day 7 days a week.
